Online 2010 Spring League Membership Application and Liability Waiver/Form

The MSL is going in GREEN! All playing and non-playing membership registration will be paperless this year. However, Every individual applying for 2010 Membership, regardless of membership type, must go online and complete the online registration process – NO EXCEPTIONS. This information will be used to track our membership for this year as well as developing team rosters. Even if your manager manually adds you to the team roster, you must go online and complete the online registration. All information will be kept in the strictest of confidentiality

Your membership information will then be transferred to your Team Roster. This will be e-mailed to your manager/coach with parts of the information that you included on your online registration. Your manager/coach will be responsible for obtaining your signature on this form. Verify the information and make corrections as necessary prior to signing.

Complete the 2010 Spring League Membership Application and Liability Waiver/Form online. After you submit the form, there is nothing for you to sign, because you willl sign the waiver electronically.

Payment Process

Each member who is required to pay membership dues (playing member, non-playing member, and sponsor playing member) may do so via the following methods:

1. Pay Online through the MSL Website via GooglePay. If you choose this option, review the instruction guidelines posted. It is important that you indicate what team you are playing on so we can cross-reference the payment with the proper team. The instructions indicate the recommended methods to post your payment. We DO NOT see your credit card information, so don’t worry about that. Online check payments are not available at this time.

IMPORTANT: Please read the GooglePay Checkout Instructions before submitting your online payment. We need to capture your team name on the application form.

To include your team name on the online form:

1. For My shipping address is, click A different address.
2. In the Recipient's name box, type your name.
3. In the Shipping Address box, type your team name.

2. Make payment to your team/coach/manager. If you opt not to pay online, we prefer that you make payment directly to your team, coach, or manager and that they write one check for membership dues that were not paid online. We recommend that you pay via check, cashier’s check or money order so that you can validate that payment was made.

3. Make alternate payment to the MSL. If you opt not to pay online, you may make payment directly to the MSL in the form of Personal Check, Cashier’s Check or Money Order made payable to the Montrose Softball League. WE DO NOT ACCEPT CASH PAYMENT. Any returned checks for NSF/Non-Payment will be subject to a “Returned Item Fee.”

Once your registration is complete and your dues received, your application process is complete and will be reviewed by the Board of Trustees. If we have any questions we will contact you from the information provided. Should your application be denied or revoked, your dues will be refunded. Once the season has begun, should your membership be suspended or you leave the League, your dues will NOT be refunded.
